The "Vacu <br /> Tect" test report indicates the leak detector associated with the 10,000 <br /> gallon super unleaded tank failed the leak detection test. The general <br /> 9 P <br /> comments section of this test report says, Replaced Leak detector on SUN <br /> product". When the approved monitoring system associated with an <br /> underground storage tank is changed, a PHS-EHD permit application, <br /> (enclosed) and a $234.00 permit fee are required to be submitted to PHS- <br /> EHD.
