Ham Lane, Lodi <br />I have reviewed the installation application submitted to this <br />department for the above referenced facility. The following <br />items need to be addressed prior to permit approval. <br />1. The responsible party (Ken Paige) to be billed for <br />additional PHS-EHD staff time expended beyond the 8 hour <br />minimum installation payment needs to acknowledge this <br />responsibility for additional billing by signature and date <br />on page 3 of the application. <br />2. The application specifies the manufacturer for the tanks to <br />be installed is Xerxes, page TL6 of the blueprints indicates <br />Trusco\Xerxes, and page TL7 indicates Xerxes or Owens <br />Corning. You need to specify what equipment (tanks, piping, <br />monitoring panel, probes, leak detectors..... etc) will be <br />installed at this facility and submit material specification <br />sheets and manufacturer's brochures for the equipment. <br />3. A Monitoring and response plan according to section 2636 (d) <br />(1) & (2), CCR, Title 23 Waters, Division 3, Chapter 16, <br />needs to be submitted. <br />4. The owner operator (Ken Paige) needs to submit to this <br />office, evidence of financial responsibility as required by <br />section 2807, Chapter 18, Division 3, Title 23, CCR. <br />5. In the event contamination is observed, confirmed or <br />suspected as a result of a leaking UST system it is the <br />responsibility of the owner or operator to submit a workplan <br />to PHS-EHD Site Mitigation Unit prior to initiating any <br />assessment or remediation activities. To minimize delays, a <br />Corrective Action Plan may be submitted concurrent with tank <br />closure or installation plans. Address one copy of the <br />Corrective Action Plan to the Site Mitigation Unit for <br />review.
