Ham Lane, Lodi <br />I have reviewed the underground storage tank removal application <br />for the above referenced facility. The following items need to <br />be clarified before permit approval. <br />1. If the integrity of the monitoring wells are jeopardized <br />from the tank removal activities, then they shall be <br />repaired or destroyed under PHS-EHD permit and inspection. <br />2. The laboratory name and phone number needs to be identified <br />on page three of the application. <br />3. Leaded gasoline constituents will be the required laboratory <br />analysis for the 5,000 gallon tank and both 10,000 gallon <br />tanks. <br />4. The capacity of the waste oil tank is 1,000 gallons and will <br />require a minimum of two soil samples. <br />5. The dry ice requirements for gasoline tanks is 30lbs/1,000 <br />gallon capacity. <br />6. Soil samples will be taken two to four feet below the bottom <br />of the tanks. <br />7. The applicable portions of section four, page ten needs to <br />be completed. <br />8. Please submit a "Site Health & Safety Plan" to address all <br />potential hazards for this specific job site. <br />9.
