Lodi, CA 95242 SR0065393 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the revised scope of work of the permit application for installing a Red Jacket FX1 DV leak <br /> detector and Type H Relay 7000 on the underground storage tank (UST) system at the above <br /> noted address. The revised scope of work adds a cold start or the TLS monitor and replacement <br /> of the TLS monitor battery after the TLS monitor crashed. A monitoring certification of the UST <br /> system was witnessed by the inspector on 8-21-12. The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board (may be issued to the installation company), or a <br /> Tank Tester's license issued to the individual by the State Water Board, and <br /> b. A valid, current California UST Service Technician certificate issued by the <br /> International Code Council (ICC), and <br /> c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being tested, repaired, or serviced. <br /> 4. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site who possesses all of the following: <br /> a. A valid, current contractor's license issued by the Contractors State License Board, <br /> and <br /> b. A valid, current certificate of training from the manufacturer(s) of the component(s) <br /> being installed, and <br /> c. A valid, current UST Installation/Retrofitting certificate issued by the International <br /> Code Council (ICC). <br />
