The designated operator failed to document all the alarms from the attached alarm history on the <br /> March 2014 designated operator monthly inspection report and failed to check that they were responded <br /> to appropriately. The missing alarms include: the L3 diesel turbine sump fuel alarm on 2-27-14. During <br /> the monthly inspection, the designated operator shall review the alarm history for the previous month, <br /> check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports. <br /> 205. The June 2013 designated operator monthly inspection report was not found on site. Designated <br /> operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and <br /> ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit a copy to the EHD by Apri110, 2014. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by April 10, 2014. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br /> Inspector.
