See attached"Continuation—Official Inspection Report"for violation(s)not listed above. <br /> SUMMARY OF VIOLATIONS <br /> CLASS 1,CLASS II,or MINOR-Notice to Comply) <br /> On site to witness the annual monitoring system certification testing and conduct routine UST inspection: <br /> Class I I Violations: (201, 302, 303, and 310): <br /> 201. The maintenance records for L3 diesel turbine sump fuel alarms from 11-30-2012, 11-28-2012, <br /> 12-18-12, 12-25-12, and 12-26-12 were not found on site; according to the designated operator's <br /> monthly inspection reports dated December 4, 2012 and January 4, 2013, respectively, in all instances <br /> the water in the diesel sump was due to rain and was removed. These maintenance records illustrating <br /> how these alarms were responded to and how the removed liquid was disposed off shall be kept on site <br /> for at least 3 years. Immediately locate and maintain these records for the above L3 alarms on site and <br /> submit copies to the EHD by July 24, 2013. REPEAT VIOLATION. <br /> 302. The 91/diesel annular sensor failed to activate an audible and visual alarm when tested. The <br /> interstitial space shall be monitored with a monitoring system connected to an audible and visual alarm <br /> system. During the inspection the technician cleaned the sensor, retested it several times, and it <br /> passed. Corrected on site. <br /> 303. The diesel turbine sump sensor was located approximately 4-inches up the tank saddle and off the <br /> lowest point of the sump and not located to detect a leak at the earliest opportunity. Monitoring <br /> equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. During the <br /> inspection the technician repositioned the sensor to the lowest point of the sump. Corrected on site. <br /> REPEAT VIOLATION. <br /> 310. Approximately '/2-cup of liquid was observed in the 91 turbine sump. If water could enter into the <br /> secondary containment by precipitation of infiltration, it must be removed and disposed of properly. <br /> During the inspection the technician removed all the liquid and retained it for future re-use.
