CA 95240 Amended SR0065079 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the permit application for installing an OPW 71 SO drop tube for the 91 product overfill <br />prevention on the underground storage tank (UST) system at the above noted address. The <br />EHD has received the amendment to include rewiring of the Veeder Rod -794380.206 i.2 91 <br />sump sensar. Functionality of the rewked sensor was verified during the monitoring system <br />certification testing an 6-6-12. The permit has been approved vwth the follaving conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Cortrol <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD far approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Cortact Ads Cacapit at (209) 468-3453 to arrange the folkwing inspection appointments <br />(a 484nour notice is required for inspection appointments). <br />a. Overfill prevention verfication: Measurement of drop tubes to verify 95% tank volume <br />shut off. <br />b. Spfi bucket lest: Verily minimum 5 -gallon capacity and pass a one hour hydrostatic <br />test for equivalent). <br />Please be awarereceive that the facility cannot recve fuel in the 91 tank prior to the EHD <br />Inspe=n and approval. <br />4. Qualified service technicians must possess or work under the daect and personal <br />supervision of an individual physical present at the work site vdro possesses aN of the <br />foliardng- <br />a One of several permissible valid, current contractors licenses issued by the <br />Contractors State Ucwm Board (may be issued to the installation compary), ora <br />Tank Taster's ficense issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />Idemallonal Code Council ([CC), and <br />c. A valid, current certificate of training (tarn the manufacturer(s) of the companent(s) <br />being tested, repaired, or serviced <br />
