Kettleman Ln. Lodi, CA 95242 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the annual monitoring system certification testing. <br />4. Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility immediately. <br />11. The 87 sump sensor was on the opposite side of the product line penetration and not located to <br />detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a <br />leak at the earliest possible opportunity. Technician adjusted the sensor to the same side of the turbine <br />sump as the product line penetration. Continue to ensure that the sensors are positioned at the lowest <br />point to detect a leak at the earliest opportunity. Repeat Violation. Corrected on site. <br />21. Liquid was observed in the diesel turbine sump. If water could enter into the secondary containment <br />by precipitation or infiltration, it must be removed and disposed of properly. Technician removed the <br />liquid and added it to his test water container. Continue to remove any liquid that may enter the <br />secondary containment, make a hazardous waste determination per Title 22 hazardous waste <br />regulations, and manage it accordingly. Ensure that the sumps are maintained free of liquid. Repeat <br />Violation. Corrected on site. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 7-13-11. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122). <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AJ EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />� <br />Received By: <br />I /I 'JA x� I <br />Title: <br />EHD 23-02-003 Rev 08/10/10 CONTINUATION FORM <br />
