Kettleman Ln. <br />Lodi, CA 95240 <br />Mr. Cacapit, <br />As per your inspection on 2/18/14, the following Corrective Actions have taken place: <br />• The Designated Operator (Service Station Systems) was notified of the incorrect dates for the Monitor Certification on <br />the Designated Operator Inspections for the November and December inspections. These inspections will be verified <br />with the correct test dates on friday March 21, 2014 and at that time, the Designated Operator will correct the dates <br />on those Inspections and correctly note them on those inspection forms. <br />• The manifests noted onthe inspection Report (004664178FLE and 004664179FLE) were unintentionally damaged <br />during transit. There have been other manifests created for those damaged manifests with documentation from our <br />waste hauler stating the need for new manifests for the collected waste.
