Lodi Lodi <br />95242 <br />County Health Permit/ Approval for proposed upgrades/ modifications to be performed <br />Dear Mr. Naidu/Haz-Mat UST Plan Check, <br />Per our phone conversation, attached please find two additional sets of Service Request <br />Form, Application for UST Retrofit or Piping Repair Permit & Check #1162 for $750.00 <br />dollars ($375.00 dollars per site) we would like to respectfully submit to your office to go <br />along with documents previously submitted & in order to obtain County Health <br />Permit/ Approval for proposed Replacement of Existing Fuel Dispensers, Installation of <br />New Conversion Frames for New Dispensers, Disconnect & Reconnect Existing <br />Electrical for New Dispensers, Upgrade of Veeder-Root Sofware Version to 1.05 with <br />Cold Start & Related Modifications to be performed at subject sites.
