The permit has been <br />approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Aris Cacapit at (209) 468-3453 to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). Please be aware that <br />inspections for emergency work must be scheduled within five days of permit approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />b. S. Bravo Conversion Frame: Verify proper installation of the S. Bravo conversion <br />frame and check for leaks. <br />Please be aware that the facility cannot operate the UST system tank prior to the <br />EHD inspection and approval. <br />4. If concrete will be broken near the UST and/or piping, a secondary containment test <br />must be performed on the UST and/or piping once concrete is poured in accordance <br />with the manufacturer's guidelines. <br />
