CLASS II, or MINOR -Notice to Comply) ___ <br />On site to witness the annual monitoring system certification testing and conduct a routine inspection. <br />204. The designated operator failed to check and note the correct test dates for the monitoring system <br />certification, spill bucket, and leak detector tests on the 12-16-13 and 11-5-13 designated operator <br />monthly inspection reports. The dates for these tests were listed as 2-1-13. These test dates should <br />have been listed as 2-21-13. During the monthly inspection, the designated operator shall check that all <br />required testing and maintenance for the UST system have been completed and document the dates <br />they were done. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by March 18, 2014. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Title: <br />EHD 23-02 Rev 04/19/12 P Page 3 UST INSPECTION REPORT <br />
