Sent by Email and Certified Mail <br /> SAN JOSE, CA 95112 <br /> RE: MOBIL, 401 KETTLEMEN LANE, LODI SR0078774 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br /> application for a permit to replace a line leak detector on the 87 and 91 octane STP sumps with VMI 99 <br /> LD-2000 on the Underground Storage Tank (UST) system at the above noted address. This work was <br /> completed on February 18, 2018 and testing requirements were fulfilled on 02/23/2018, no further <br /> inspections are required for this retrofit permit. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Cesar Ruvalcaba at (209) 953-6213 to arrange the following appointments (a 48-hour <br /> notice is required for inspection appointments). Please be aware that inspections for emergency <br /> work must be scheduled within five days of permit approval. <br /> • A line leak detector test on the 87 and 91 octane UST systems was performed on <br /> 02/23/2018 during the annual Monitoring System Certification with inspector onsite. Line <br /> leak detectors detected a leak of 3 gallons per hour at 10 psi. <br /> 4. If concrete will be broken near the tank a secondary containment test must be performed on the <br /> affected USTs once concrete is poured in accordance with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible valid, <br /> current contractor's licenses issued by the Contractors State License Board (may be <br /> issued to the installation company), or a Tank Tester's license issued to the individual by <br /> the State Water Board, and <br /> b. Possess or work under the direct and personal supervision of an individual physically <br /> present at the work site who possesses a valid, current California UST Service Technician <br /> 1868 E.
