UST Facility Operating Permit Application and UST Tank Information are not current in CERS. Any <br /> change of information must be updated in CERS within 30 days of the changes. Immediately log into <br /> CERS, update the required information, and submit for review by the EHD. Complete a separate tank <br /> form for each tank. <br /> 303. The 87 East turbine sump sensor was raised approximately 3 inches above the lowest point of the <br /> sump and the dispenser 9/10 sensor was raised approximately half an inch above the sump bottom. <br /> Neither sensor were located to detect a leak at the earliest opportunity. Monitoring equipment shall be <br /> maintained to be able to detect a leak at the earliest possible opportunity. During the inspection the <br /> technician repositioned the sensors to the lowest point of the sump to detect a leak at the earliest <br /> opportunity. <br /> 306. A fuel leak was found at the diesel filters in dispensers 3/4, 5/6, and 11/12. All primary containment <br /> must be product tight. Immediately have a properly licensed, trained, and certified contractor stop the <br /> leak. If the failed component can't be replaced immediately, there is a possibility that the diesel UST <br /> system may be red tagged to prevent fuel inputs. <br /> 310. Liquid was observed in the 87 west fill and turbine sumps, the 87 east fill sump, and the diesel <br /> turbine sump. If water could enter into the secondary containment by precipitation or infiltration, it must <br /> be removed and disposed of properly. Immediately remove this liquid, make a hazardous waste <br /> determination per Title 22 hazardous waste regulations, and manage it accordingly. Ensure that the <br /> secondary containment sumps are maintained free of liquid. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by August 24, 2013. <br /> Ensure that all required information has been entered into the online California Electronic Reporting <br /> System. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector: Received By: �\ � Title: <br /> EHD 23-02 Rev 04/19/12 Page 3 UST INSPECTION REPORT <br />
