A current Designated Operator notification was not provided to the EHD within 30 days of the <br /> change. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD <br /> identifying all the designated operators for this facility within 30 days of receiving this report. <br /> 313. The East 87 spill container failed when tested. All spill containers shall have a minimum capacity of <br /> five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. Immediately <br /> discontinue deposition of petroleum into this tank until the component is repaired or replaced by a <br /> properly licensed, trained, and certified contractor under permit and inspection of the EHD. If the East <br /> 87 spill container can't be replaced immediately, there is a possibility that the UST system may be red <br /> tagged to prevent fuel inputs. <br /> Notes <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, to the EHD within 30 days of <br /> receiving this report. <br /> Felix Ramirez (Technician) changed the battery on the TLS350 prior to performing positive Shutdown. <br /> No permit is required. <br /> Facility has 2 87 tanks, East 87 tank operates dispenser 3/4, 5/6, 11/12, and West 87 tank operates 1/2, <br /> 7/8, 9/10. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANYTIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector.
