Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />On site to conduct the routine inspection and witness the annual monitoring system certification testing. <br />4. Financial responsibility documents on file are dated 6-10-10. A current copy was not submitted to the <br />EHD by 6-10-11. Current financial responsibility documents are required to be submitted annually. <br />During the inspection the owner submitted a current copy of the financial responsibility documents dated <br />7-2-11. Ensure that the financial responsibility documents are completed and submitted annually. These <br />documents are next due by 7-2-12. Corrected on Site. <br />17. The 87 #2(East) line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. The 87 #2(East) line was purged of air and the <br />87 #2(East) line leak detector was retested to pass during the inspection. Corrected on site. <br />21. Liquid was observed in the the 87 #1 (West), the 87 #2(East), and the diesel turbine and fill sumps. If <br />water could enter into the secondary containment by precipitation or infiltration, it must be removed and <br />disposed of properly. Owner stated that it was condensation water. Technician removed the liquid and <br />placed it into the oil water separator. Ensure that the turbine and fill sumps are maintained free of liquid. <br />Corrected on site. <br />23c. The designated operator failed to attach the alarm history report on the 7-15-11 designated <br />operator monthly inspection report for the Q5 diesel product line leak detector. The designated operator <br />failed to attach the alarm history report on the 8-13-10 designated operator monthly inspection report for <br />the L14 dispenser 5-6 and product line leak detectors: Q1 91, Q2 87 #2(East), Q3 87 #1 (West), and Q5 <br />diesel. During the monthly inspection, the designated operator shall review the alarm history for the <br />previous month, check that each alarm was documented and responded to appropriately, and attach a <br />copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. <br />26. The 87 #1 (West) and #2(East) spill container testing was last performed on 7-9-09 and was <br />completed on 8-13-10, one month past due. These tests are required once every 12 months.
