Unfortunately, <br />this documentation does not meet the requirements set forth in Title 23, Underground Storage <br />Tank Regulations. In order for this office to verify training, you must submit the actual training <br />record that states what topics have been covered, the names of the employees that have been <br />trained, and that is signed by the Designated Operator. <br />Please remit the requested information to my attention.
