Maintenance and monitoring records for the last three years were not found on site. The 2-17-15 designated operator report did not have a maintenance receipt to address the Q2 87(2) shut down alarm on 2-16-15 nor for the T1 91 ATG probe out alarm on 8-15-14. Also, the designated operator and facility personnel did not document these alarms. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. 207 CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not completed. The designated operator failed to check and note the correct test dates for the secondary containment tests on the 7-8-15, 6-19-15, 5-15-15, 4-17-15, 3-27-15, 2-17-15, and 1-27-15 designated operator monthly inspection reports. On the 7-8-15 and 6-19-15 designated operator reports, the designated operator listed the secondary containment test as 1-20-15. On the 5-15-15, 4-17-15, 3-27-15, 2-17-15, and 1-27-15 designated operator monthly inspection reports, the designated operator listed the secondary containment test as 1-2-15. This test date should have been listed as 1-28-15. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. The 2-17-15 designated operator report did not have a maintenance receipt to address the Q2 87(2) shut down alarm on 2-16-15 nor for the T1 91 ATG probe out alarm on 8-15-14. Also, the designated operator and facility personnel did not document these alarms. The designated operator failed to document all the alarms from the attached alarm history on the 2-17-15 and 9-17-14 designated operator monthly inspection reports and failed to check that they were responded to appropriately. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class Il. 326 CCR 2635(b)(2)Overfill prevention system not present, was overridden, or failed to meet overfill requirements. The external visual alarm used as part of the overfill prevention system is not currently functional on the diesel UST. The external visual alarm was replaced by the business owner Harvinder Singh and retested by Afford-a-Test during the inspection. This was corrected on site. This is a Class II violation.
