All appointments shall be made 48 hours in advance. <br />2. The installer of the tanks and piping must be adequately trained as <br />evidenced by a certificate of training issued by the tank and piping <br />manufacturers (as required in the California Underground Storage Tank <br />regulations, section 2635(d) (1)). A copy of the appropriate certifications <br />from the tank and piping installer shall be submitted to this office before <br />the installation begins. <br />3. State Water Resources Control Board Form "C" must be completed and <br />submitted to this office before an operating permit will be issued. <br />4. A tank integrity test shall be conducted after installation when the station <br />is operable, but before the underground storage tanks are placed in <br />service. <br />5. A copy of this plan must be kept on site at all times. <br />6. Note that less than a 1.25 recommended safety factor has been <br />provided for the floatout and anchorage calculations for this installation. <br />Because the 1.25 safety factor is a recommended industry standard and <br />not a legal requirement, PHS-EHD will not prevent the installation of <br />these tanks with the existing designed overburden, but PHS-EHD will <br />not accept any responsibility if these tanks were to be uplifted due to <br />high water levels. <br />A Division of San Joaquin County Health Care Services <br />
