The following items are required to be submitted to this office <br />before final approval of the plan will be considered. <br />1. Please complete all portions of page four. <br />2. Please complete all portions of page five. <br />3. Please complete all portions of page seven including # 16 and # 17 A <br />4. Complete #25 on page eight. <br />5. Please submit a monitoring and response plan written in accordance with <br />section 2632 (d)(1) & (2), CCR, Title 23 Waters, Division 3, Chapter 16. <br />6. Is the American containment turbine containment vessel compatible with the <br />modern welding UST? <br />7. The site plan shows one of the UST's compartmentalized into two 5,000 gallon <br />tanks. Is this configuration planned to be installed? If it is you need to submit <br />a cross sectional diagram illustration the tank and piping system including <br />secondary containment, overflow protection equipment from below grade to <br />finish grade. <br />8. Please identify all equipment illustrated on the cross sectional diagram of the <br />proposed UST. <br />9.
