Lodi, Ca. <br /> The following documentation was required to be submitted to this office within 30 <br /> days of the tank removal date (see page two of the removal permit): <br /> a) Analytical results, mailed directly from the laboratory to PHS-EHD, <br /> including QA/QC reports and chain of custody for the 7/2/96, 7/3/96 and <br /> the 9/13/96 sampling events. <br /> b) U.S.T. Tracking record sheets for the tanks removed from this site on <br /> July 2, 1996. <br /> C) Piping disposal receipts for the product lines associated with the <br /> U.S.T.'s <br /> d) Hazardous Waste Manifests for the rinsate generated from cleaning the <br /> tanks. <br /> This site can not be considered for final closure until the above items are submitted <br /> for review.
