The following items need to be addressed prior to permit <br />approval. <br />1. You -• to obtain <br />office.,/2. Please submit a copy of your "Hazardous Substance Removal Certification" to <br />this <br />✓ 3. Please submit a "Site Health and Safety Plan" to this office. <br />4. You need to complete all applicable portions of page ten. <br />5. The service request form submitted with the closure application indicated L&M <br />Construction as the billing party. Page six of the closure application indicated <br />Amrik Singh as the responsible party for billing. Who is the responsible party <br />to be billed for additional PHS-EHD staff time expended beyond the permit <br />payment? <br />./6. In the event contamination is observed, confirmed or suspected as a result of a <br />leaking UST system, it is the responsibility of the owner or operator to submit a <br />workplan to PHS-EHD Site Mitigation Unit prior to initiating any assessment or <br />remediatiion activities. To minimize delays, a Corrective Action Plan may be <br />submitted concurrent with the tank closure plan.
