Lockeford St. Lodi, CA 95240 SR0073734 <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the application for a permit to install an OPW drain valve on the direct bury 87 spill container, <br /> locate and fix the diesel leak from the filter in dispenser#3/4, and pull back the boot on the <br /> secondary line in the diesel submersible turbine pump sump on the underground storage tank <br /> (UST) system at the above noted address. The permit has been approved with the following <br /> conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Aris Veloso at (209) 468-3453 to arrange the following inspection appointments <br /> (a 48-hour notice is required for inspection appointments). Please be aware that <br /> inspections for emergency work must be scheduled within five days of permit approval. <br /> a. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test (or equivalent). Proper drainage must also be verified. <br /> b. Visual inspection: Verify that the diesel filter in dispenser#3/4 is not leaking. <br /> c. Visual inspection: Verify that the test boot on the diesel product line is no longer <br /> affixed in the testing position. <br /> Please be aware that the facility cannot receive fuel in the diesel tank prior to the <br /> EHD inspection and approval. <br /> 4. If concrete will be broken near the UST and/or piping, a secondary containment test <br /> must be performed on the UST and/or piping once concrete is poured in accordance <br /> with the manufacturer's guidelines. <br /> 5. Any individual performing the work of a service technician must meet the following <br /> qualifications: <br />
