GENERAL INFORMATION: <br />Obtain an <br />EPA Site Number from the Department of Health <br />hazardous waste removal activities associ�ted with under Services (916) 324-1781 for temporary <br />ground storage tank (UST) removals. <br />Disposal Information: public Health Services Tracking <br />Sheet shall accompany each <br />from site. <br />Tank(s) will be issued an identification tank removed <br />on the number which pHS_EHD.s <br />representative will note <br />Tank Tracking Sheet Contractor will affix same identification number onto tank end using <br />fluorescent spray paint.
