Enclosed are the required state forms for <br />underground storage tank permits. If San Joaquin County requires additional information, please <br />send me the appropriate forms at your earliest convenience. UST permits and fee invoices should <br />be sent to the address listed on the Form A and can also be addressed to myself. <br />Also enclosed is a Facility Information form that I requested from your office. At the time I <br />requested this form, I did not realize that your office had updated information regarding the <br />ownership of the site. I have not added any information or remitted payment with this form, as it <br />is already accurate.
