The diesel line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, <br /> and certified contractor repair or replace the failed leak detector(LG 113-x listed, if applicable) and <br /> obtain a permit within one business day from the EHD. If the failed leak detector can't be replaced <br /> immediately, there is a possibility that the diesel UST system may be red tagged to prevent fuel inputs. <br /> 21. Liquid was observed in the #5 UDC sump. If water could enter into the secondary containment by <br /> precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this liquid, <br /> make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it <br /> accordingly. Ensure that the#5 UDC sump is maintained free of liquid. <br /> 23c. The designated operator failed to check and note the correct test dates for the secondary <br /> containment, line tightness test, leak detector on the October 2009-August 2010 designated operator <br /> monthly inspection reports. During the monthly inspection, the designated operator shall check that all <br /> required testing and maintenance for the UST system have been completed and document the dates <br /> they were done. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by October 2, 2010. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> �/vy%� <br /> EHD 23-02-003 - 10 CONTINUATION FORM <br />
