950 W 111h St, Tracy A <br />E <br />c. A valid, current certificate of training from the manufacturer(s) of the <br />component(s) being tested, repaired, or serviced, and <br />d. Each person installing or testing any component must hold a certificate of <br />training from the manufacturer of that component. <br />5. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br />date is required, a written request must be submitted to this office prior to the expiration <br />date. This permit expires September 26, 2009 but does not grant an extension to the <br />ARB EVR upgrade deadline of April 1, 2009. <br />6. All components must be certified by an independent testing organization and labeled as <br />required by the organization. (UL, ULC, ANSI, API, ASTM, etc. See California Code of <br />Regulations (CCR), Title 23 Chapter 16 Table B for complete list). The component must <br />bear the appropriate mark as required by the independent testing organization. <br />7. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work.
