Secondary containment testing was last performed on 05/02/2008 and was due by May 2011. <br /> Testing has not been completed and is three months past due. Secondary containment testing is <br /> required once every 36 months. Immediately schedule this test and provide 48 hours notification to the <br /> EHD. Submit test results within 30 days of testing. <br /> 21. Moisture and wet/dampness was felt by the technician at least two feet length of the diesel annular <br /> sensor. There was no odor, oily texture nor visual color appearance of fuel on the wrap around diesel <br /> annular sensor. There could be a high possibility that there is liquid in the diesel annular space. Per <br /> technician, there could be approximately one cup of liquid. In addition, there were two L4 diesel annular <br /> sensor out alarms on July 22 and June 6, 2011 which both had cleared itself which could be an <br /> indication that liquid could have triggered the sensor momentarily. If water could enter into the <br /> secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste <br /> regulations, and manage it accordingly. Investigate the source of how the liquid could enter into the <br /> diesel annular space. Ensure that the all annular space is maintained free of liquid. Submit a copy of the <br /> work order or supporting document that liquid was removed from the diesel annular space. <br /> 23c. The designated operator failed to check and note the correct test dates for the secondary <br /> containment testing on the designated operator monthly inspection reports from October 2010 to <br /> present, July 2011. The designated operator noted the secondary containment testing as 05/02/2010. <br /> The correct test date is 05/02/2008. During the monthly inspection, the designated operator shall <br /> check that all required testing and maintenance for the UST system have been completed and document <br /> the dates they were done. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. Submit a statement how to ensure the <br /> test date is correctly documented. <br /> Submit to SJC EHD within 30 days (09/19/11)- <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for each of the above violations <br /> 3. requested/supporting documentation ie. work order <br /> Notes: - Next Designated operator employee training due by 11/2011 <br /> - Provided free training schedule.
