A check in the amount of $936.00 for the removal fees. <br />4. 4 completed form "B"s. <br />In addition, we have enclosed the following information for the <br />Tank installation portion of the project. <br />1. Four complete sets of plans. <br />2. A check in the amount of $904.00 for the processing fee. <br />3. A letter of authorization for Tait and Associates to <br />sign on behalf of ARCO Products Company. <br />4. Completed form A and B's. <br />It is my understanding that an overtime plan check is available <br />and we would be interested in this.
