Contact the local fire department and San Joaquin Valley Air District for any applicable <br />permits. <br />2. Any changes shall be submitted to this office for approval prior to commencing work. All <br />new addenda supercede the previous ones. <br />3. At least one person who holds the International Code Council (ICC) UST Service <br />Technician certification and manufacturers' certificate of training for all components <br />installed or tested must be onsite when work is performed. <br />4. Each person installing or testing any component must hold a certificate of training from <br />the manufacturer of that component. <br />5. EHD must witness the functionality of the new sensor to ensure that monitoring meets the <br />requirements of California Code of Regulations Title 23 and the Health & Safety Code. <br />Allow 48-hour notice for all appointments. <br />6. Complete and submit a copy of a UST Installation Certificate of Compliance (Former <br />Form C) for the 91 tank no later than 30 days after work is completed. <br />7.
