The following describes the violation items you <br /> noted during your inspection as well as corrective actions taken: <br /> Item #206—Work order#192585 was found on site describing the replacement of L5 Regular <br /> center STP 208 sensor on 10/25/2017 and EHD was not notified of this replacement. <br /> On 9/12/2018, BP testing and maintenance vendor, Gettler-Ryan, Inc., submitted a permit <br /> application and associated fees for the replacement of the sensor. On 9/21/2018, your <br /> department approved the permit under SR0079679. A copy of the permit package is enclosed <br /> herein for your records. <br /> Item #309 — The L11 Regular West STP sensor failed to activate an audible and visual alarm <br /> when tested. <br /> On 8/30/2018, BP testing and maintenance vendor, Gettler-Ryan, Inc., replaced the sensor and <br /> retested. This item was corrected onsite. A copy of the monitoring system certification is <br /> enclosed herein for your records. <br /> Item #314 — The 3/14/2018 secondary containment results indicate failure at the T2 regular <br /> pipe. <br /> On 3/16/2018, BP testing and maintenance vendor, Gettler-Ryan, Inc., made repairs and on <br /> 3/27/2018, retested the pipe with passing results. Per our correspondence, dated 9/11/2018, <br /> you will accept the passing results from 3/27/2018. A copy of the test results are enclosed <br /> herein for your records. <br /> It is our policy to operate in full compliance with all laws and regulations at our retail facilities. <br /> However, when we fall short of perfection, we endeavor to immediately address deficiencies, issues of <br /> concern, and/or agency requests regarding our retail facilities.
