The following <br />describes the requested action you noted during your inspection as well as corrective actions taken: <br />* #5&6 Monitoring program (Monitoring and Response Plans) must be current, approved by this office & a <br />copy kept onsite. At time of inspection this office did not have a current approved copy of Monitoring <br />program & I could not be located onsite. Within 30 days (3.21.08) submit the monitoring program to this <br />office for approval. Once signed a signed copy will be returned to be kept on site. Karen (Operator) had <br />submitted a copy of the original plan which was maintained on site and I am also including a copy <br />of the original plan. Additionally, according to Steve Skanderson who spoke with you on 3/19th, <br />an agreement was made to update the plan utilizing the new State Forms which includes the <br />installation of the new sensors and other changes you discussed with Steve. I have signed these <br />forms and a copy is attached for your files. <br />* #12 87 STP Sump sensor went into alarm for a low liquid alarm but failed to recognize a high liquid alarm. <br />All monitoring equipment must be functional at all times. ImmediatelyrepOpelrepair th-i-s-sensor under <br />pern*firom this office. We were originally requesting an inspection extension because we did not <br />have the approved permit back yet. We have received the approval and installation will occur this <br />Friday per your agreement with Debbie Watts at SSS. <br />* #23 Designated Operator Dennis Smith performed monthly inspection for the month of April 2007. Dennis <br />Smith is not listed as a Designated Operator for this site. Any Designated Operator who performs monthly <br />inspections must be listed on the statement for UST Owner w/in 30 days of change. Take all necessary <br />precautions in the future to prevent this from happening again. Attached is a letter from GVR regarding <br />this specific situation - please see the attachment for specifics. <br />* #32 87 STP Sump sensor failed during MC obtain pqrmhtfrom this office Priorto making necessa <br />repair*Wcacemept.
