Sit NAOAQVIN <br />COUNTY <br />Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />TRACY SHELL" <br />3725 N TRACY BLVD, TRACY <br />January 25, 2018 <br />SUMMARY OF VIOLATIONS <br />(CLASS 1, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />309 <br />CCR 2636(f)(1) Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow product. <br />The UDC dispenser #4 sensor failed to stop the flow of product to the dispenser when tested to ensure continuous <br />monitoring of the tank system. Technician troubleshooted the sensor in UDC #4 by removing the sensor and tested <br />the functionality of the sensor in dispenser #2. Sensor functionality was verified and shut down of the product at the <br />dispenser #2 was verified. Technician stated that the electrical connector was bad and the part needed to be <br />replaced. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br />product at the dispenser when it detects a leak. Technician replaced the electrical connector and replaced sensor <br />like for like because the electrical connectors come with the sensor and functionality of the sensor in dispenser #4 <br />was verified and the flow of product to the dispenser was shut down. A "UST Retrofit Verification with Inspector <br />Already Onsite" form has been completed and provided to the operator and <br />contractor. This was corrected on site. <br />This is a repeat violation, Class ll. <br />313 <br />HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product -tight. <br />Weepage was found at/above shear valve at dispenser #2, #8, #4, #10, and #11 during the inspection, indicating a <br />leak in the primary piping. All primary containment for the UST system must be tight. Immediately have a properly <br />licensed, trained, and certified contractor repair or replace the failed component under permit and inspection of the <br />EHD. <br />This is a repeat violation, Class ll. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by February 24th, 2018. <br />Documents reviewed by regulator: Monitoring plans, Designated operators monthly reports, monitoring system <br />certification reports (2017,2016, and 2015), Secondary containment testing report (2016), employee training log, <br />response plan, financial responsibility documents, tank owner/operator agreement statement, operating permit, <br />alarm history and sensor set up, and maintenance records. <br />Documents received by operator: routine inspection report and return to compliance certification form. <br />AFTER THE COMPLIANCE DATE, EHD WILL BILL FOR ALL TIME AND ACTIVITIES ASSOCIATED WITH BRINGING <br />THIS FACILITY BACK INTO COMPLIANCE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Received by: Date: January 25, 2018 <br />Inspector: <br />Inspector Phone: <br />Printed Name and Title: <br />BETTY HO, EH Specialist <br />(209) 468-3455 <br />Tanya Moore, Store Manager <br />FA0003780 PR0231417 SCO01 01/25/2018 <br />EHD 23-01 Rev. 10/31/2017 Page 5 of 5 Underground Storage Tank Program OIR <br />1868 E.
