• <br />0 <br />0 <br />The following is an itemized list of underground storage tank violations that have <br />been addressed for TRACY SHELL as of March 27, 2017. ZMP7 <br />Open violations from January 25, 2017 inspection <br />Violation #209 - Designated operator did not inspect all required items and/or the inspection reports not <br />completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the 11/17/16 designate <br />operator monthly inspection report and failed to check that they were responded to appropriately. The missing <br />alarms include: L12 diesel fill sump piping sump fuel alarm on 10/11/16. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was documented <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. <br />Violation #211 - Designated operator employee training not performed or log not kept. <br />The designated operator employee training for Gloria Edward was not current. The designated operator shall train <br />facility employees for which he or she is responsible in the proper operation and maintenance of the UST system <br />once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />Violation #313 - Failure to construct, operate, and maintain primary containment as product -tight. <br />Seepage was found at or above the shear valves at dispenser 7/8 and dispenser 9/10 during the inspection. The <br />service technicians removed the seepage in the two dispensers. Continue to monitor primary piping seepage in the <br />two dispensers. If seepage continues to occur, immediately contact a licensed, trained and certified contractor to <br />repair or replace the failed components. <br />Open violations from January 27, 2016 inspection <br />Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. The monitoring system alarm <br />panel test log was incomplete. These records shall be maintained on site for at least three years. Monitoring record: <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br />the last three years on site and submit copies to the EHD. <br />Violation #314 - Primary containment not tight (pre -Jul 2003). <br />Approximately one gallon of liquid was found in the 91 product fill sump, indicating a leak in the primary piping. All <br />primary containment for the UST system must be tight. Immediately have a properly licensed, trained, and certified <br />contractor repair or replace the failed component under permit and inspection of the EHD. <br />Seepage or leakage at or above the shear valve in dispensers 1/2 for 87, 89, and 91 products, 3/4 for 91 product, <br />5/6 for 91 product, 7/8 for 87 and 91 products, 9/10 for 87, 89, and 91 products, and 11/12 for 87 product were <br />detected during the inspection. The service technicians were able to correct the seepage in some dispensers; <br />however, dispensers 1/2 still has seepage for 87, 89 and 91 products, 3/4 still has seepage at 91 product, 7/8 still ha: <br />seepage at 91 product, and 11/12 still has seepage ate 87 product. Immediately have a properly licensed, trained, <br />and certified contractor repair or replace the failed component under permit and inspection of the EHD. <br />Page 2 of 2 <br />
