See attached "Continuation — Official Inspection Report" for violation(s) not listed above. <br />SUMMARY OF VIOLATIONS <br />CLASS 1, CLASS 11, or MINOR -Notice to Comply) <br />Routine Annual Inspection and Monitoring System Certification: <br />Class II violations: 310 and 311: <br />310. Liquid was observed in the 89 STP sump (approximately Y2 liter). The service technician removed <br />all the liquid at the time of inspection. The removed liquid was placed into the on-site hazardous waste <br />container, pending proper disposal. Ensure that the 89 STP sump is maintained free of liquid, if the STP <br />sump continues to accumulate liquid, have appropriately licensed, trained, and certified contractor <br />replace or repair the failed components under permit and inspection of the EHD. <br />311. The 89 line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. The service technician replaced the failed leak <br />detector with like for like (new VMI leak detector Model - 99LD-2000; Serial Number - 12021302), <br />retested, and it passed. The "UST Retrofit Verification with Inspector Already Onsite" inspection form <br />was completed and a copy was provided to the site operator. <br />Notice to Comply- Minor violation: 999- <br />999. (HSC 25185(c)(3) and HSC 25404.1.2(c)(1)): The Return to Compliance Certification (RTC) in <br />response to the Routine Annual Inspection Report, dated June 29, 2013, was received by EHD on <br />August 8, 2012. This submittal was received late, in excess of 30 days. All corrections to Minor, Class 1 <br />and Class 2 violations must be submitted to EHD within 30 days unless otherwise specified in the <br />inspection report.
