The designated operator failed to check and note the correct test dates for the secondary <br />containment testing. The designated operator monthly inspection reports between July 20, 2013 and <br />June 28, 2013 listed the secondary containment test date as September 30, 2013, the actual test was <br />conducted on September 29, 2013. During the monthly inspection, the designated operator shall check <br />that all required testing and maintenance for the UST system have been completed and document the <br />dates they were done. Ensure that designated operators performing monthly inspections at this facility <br />are including all of the required information on the reports. Designated operator corrected the past <br />reports at the time of inspection. Corrected onsite. <br />311. The 91 -octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br />capable of detecting a 3 -gallon per hour leak at 10 psi. The leak detector was replaced and retested <br />during the inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been <br />completed and provided to the operator and contractor. Corrected onsite. <br />Minor -Notice to Comply (109)- <br />109. Annual monitoring system certification, leak detector testing, and spill container testing was <br />performed on July 20, 2012 and a test report was submitted the EHD on August 16, 2012 in excess of <br />30 days. A copy of the test report must be submitted within 30 days of the tests. Ensure that future test <br />results are submitted to the EHD within 30 days of the tests. <br />Notes: Discussed CERS submittals and necessary corrections. An updated copy of Certification of <br />Financial Responsibility and Letter from Chief Financial Officer, dated March 3, 2013 was submitted to <br />the EHD at the time of inspection. A turbine motor (87 -octane STP sump) was replaced at the time of <br />inspection.
