UST facility form is lacking BOE Maintenance Fee Account Numbers or BOE number is incorrect. <br />Immediately complete and submit a copy of the facility form to the EHD. <br />6. Approved copies of the monitoring and response plans were not found on site. Copies of these plans <br />shall be retained on site at all times. Immediately locate and retain a copy on site. <br />17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br />of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and <br />certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a <br />permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, <br />there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. <br />23a. A new designated operator was hired on November 15, 2010 and notification was not provided to <br />the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. Submit <br />notification to the EHD identifying all the designated operators for this facility by August 21, 2011. <br />32. The 87 MLLD was replaced on I01 and a permit was not obtained from the EHD. <br />Emergency repairs relating to UST s stem monitoring and release detection may be performed <br />by a properly licensed, trained, and certified contractor pursuant to the EHD's policy and a <br />permit obtained within one business day from the EHD. All other repairs must have a permit <br />submitted to and approved by the EHD prior to beginning work. Immediately obtain a permit <br />from the EHD for the 87 MLLD. A "Failure to Obtain a Permit" penalty applies. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by August 21, 2011. <br />Note: Financial Responsibility is due by 8/11. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANYTIME AT EHD'S CURRENT HOURLY RATE. <br />EHD <br />e"ved By: C Title: <br />v <br />EHD 23-02-003 Rev 12/11 CONTINUATION FORM <br />
