UST facility form is lacking BOE Maintenance Fee Account Numbers or BOE number is incorrect. <br />Immediately complete and submit a copy of the facility form to the EHD. <br />6. Approved copies of the monitoring and response plans were not found on site. Copies of these plans <br />shall be retained on site at all times. Immediately locate and retain a copy on site. <br />17. The 91 line leak detector failed to detect a leak when tested. All line leak detectors shall be capable <br />of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and <br />certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) and obtain a <br />permit within one business day from the EHD. If the failed leak detector can't be replaced immediately, <br />there is a possibility that the 91 UST system may be red tagged to prevent fuel inputs. <br />23a. A new designated operator was hired on November 15, 2010 and notification was not provided to <br />the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days.
