This information was required to be submitted by 10/17/17. This information was <br /> received on 4/30/18 resulting in a non-compliant status for this facility. An operator that receives an inspection <br /> report shall have 30 days to submit a written response that includes a statement documenting corrective actions <br /> taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective <br /> actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> This is a Class 11 violation. <br /> 204 CCR 2712(1)Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these approved plans shall <br /> be accessible on site at all times. Immediately locate and retain an approved copy on site.The cashier on site was <br /> able to make the corrections to the monitoring plan while inspector was on site. <br /> This was corrected on site. <br /> This is a repeat violation, Class II. <br /> 206 CCR 2712 Failure to comply with UST operating permit conditions. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br /> This is a minor violation. <br /> 211 CCR 2715(0(2)Designated operator employee training not performed or log not kept. <br /> The designated operator employee training did not indicated the hire dates for the employees that she has trained. <br /> The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
