responsible party shall contact all water purveyors, industrial sites and private properties known or <br /> suspected to operate wells in the area and secure the locations of all water supply wells in the area. Please <br /> include prints of historic aerial photos used as part of your study,and use data tables and maps to report the <br /> data collected as part of your survey. <br /> Please note that it is your responsibility as the discharger to protect and notify consumers of <br /> potential water quality issues related to this release. <br /> In the event elevated and variable MTBE concentrations are observed we request that you immediately <br /> characterize the ongoing sources of dissolved phase MTBE contamination at your site. Please report the <br /> results of your characterization in the SWI Report and evaluate cost effective clean up methods during the <br /> SWI phase. We also request that you perform a tank system tightness test and visually inspect all <br /> underground storage tank (UST) sumps and other appurtenances to determine if your UST system is <br /> currently leaking. Please determine (1) the approximate time frame of the MTBE/Additives release first <br /> occurring at your site; (2) the use history of MTBE/Additives at your site; and (3) the history of all <br /> unauthorized releases and spills at your site. Report your findings in the SWI Report requested below. <br /> Additionally, if you have any MTBE, other fuel additives, or any other data previously collected at your <br /> site that has not been submitted to PHS/EHD, you are requested to submit this data immediately and <br /> update cumulative data tables accordingly. <br /> We request that you monitor the groundwater contaminant plumes on a quarterly basis. Additional <br /> wells will be required to define the extent of the plume if it continues to migrate. Please discuss the results <br /> of your plume monitoring in your Quarterly Reports. In the event continued plume migration is identified, <br /> please include recommendations for the installation of additional wells and cost effective remediation <br /> strategies in the Quarterly Reports requested below. <br /> Please submit technical reports to the San Joaquin County Public Health Services Environmental Health <br /> Division(PHS/EHD),Attention: Ron Rowe, according to the following schedule: <br /> • 30 days from date of this letter-Work Plan Addendum for completion of SWI <br /> • 45 days from work plan submittal-Soil and Water Investigation(SWI)Report <br /> • 210 days from work plan submittal-Corrective Action Plan <br /> We request that all required work be performed in a prompt and timely manner under the direct supervision <br /> (witnessed)of PHS/EHD staff. We have proposed a schedule for the submittal of the SWI Report and the <br /> CAP.
