The permit has been approved with the following <br />conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />priort.commencing work. All new • id. - •supersede previous ones. <br />3. Contact Muniappa Naidu at (209) 468-3439 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). Please be <br />aware that inspections for emergency work must be scheduled within five days of permit <br />approval. <br />a. Sensor functionality: Functionality of the monitoring system must be verified to <br />ensure that monitoring has been restored to meet the requirements of Title 23 <br />California Code of Regulations and the Health & Safety Code. A copy of the alarm <br />history shall be printed prior to performing the "cold start" of the Veeder Root system <br />and made available at the time of EHD inspection. <br />-'-,'lease be aware that the facility cannot operate the UST system prior to the EHD <br />.nspection and approval. <br />4. If concrete will be broken near the UST and/or piping, a secondary containment test <br />must be performed on the UST and/or piping once concrete is poured in accordance <br />with the manufacturer's guidelines. <br />5. Provide or make available certification for all additional employees and subcontractors <br />that assist in the upgrading process. <br />
