Plan is incomplete. Submit new completed closure pian. <br /> 2. Pages 3,6,9, of the closure plan must be signed by the owner of the property. <br /> 3.The site safety plan lacks map to the hospital with shortest route clearly marked. <br /> 4.The site safety plan indicates that the tanks are former heating oil tanks.The plan also indicates that <br /> the tanks are diesel.This Departments records indicate that the site is equipped with gasoline tanks <br /> only. Please clarify. <br /> Installation Plan: <br /> 1. The plan lacks over-burden calculations. Provide. <br /> 2. The plan indicates that APT piping will be used.This Department is not familiar with this type of <br /> piping. Provide manufactures information sheets on the piping. <br /> 3.The plan indicates that Amcor dispenser pans will be used. Provide manufactures information sheets. <br /> 4. The plan is vague. Provide more detail in the dispenser sump construction and monitoring equipment <br /> location. <br /> 5. The plan indicates that the tank has only one striker plate per tank. Title 23 Section 2631 states that <br /> the primary tank must have striker plates installed below every accessible opening. <br /> 6. All piping must slope back to the monitoring sump at least 1/8 inch per foot. <br /> 7. Piping runs not called out.The product and vapor must be indicated. <br /> 8. The plan does not indicate weather the piping will be monitored using the positive auto shut off <br /> exemption.
