Also, please note that dontinued use of the <br />certificate is contingent upon; <br />successful completion of the renewal site visit; <br />acceptable performance in the required performance evaluation (PE) studies; <br />timely payment of all fees, including an annual fee due 04/30/1999; <br />compliance with Environmental Laboratory Accreditation Program (ELAP) statutes <br />Section 100825, et seq.) and Regulations (California Code of Regulations <br />Title 22, Division 4, Chapter 19). <br />An updated "List of Approved Fields of Testing and Analytes" will be issued to the laboratory upon <br />completion of the renewal process. The application for the next renewal must be received 90 days <br />before the expiration of this certificate to remain in force according to the CCR, Section 64801 through <br />64827. <br />r -MI M1.
