Wilson, R.E.H.S. <br />Margaret Lagorio, R.E.H.S. <br />Robert McClellon, R.E.H.S. <br />Mark Barcellos, R.E.H.S. <br />San Joaquin County Environmental Health Department (EHD) performed a routine underground storage tank <br />inspection on August 27, 2002 at the above referenced site. <br />An inspection report was issued to you along with a request for pertinent information that was necessary in order to <br />bring this site into compliance. You were required to submit this information by September 27, 2002. <br />Failure to correct the violations will result in continuing/additional violations. Failure to submit the completed <br />underground storage tank information by December 27, 2002 will result in further legal action. <br />The missing information include(s); <br />Current State Form A <br />Current State Form B <br />Cf Monitoring and Response Plan <br />�' Annual Line Tightness Test Results <br />Submit Certification of financial responsibility <br />and letter from the chief financial officer <br />Annual Line Leak Detector Results <br />Annual Monitoring Certification <br />[ Maintenance and Monitoring Log <br />Penrrit submitted to repair/replace failed equipment <br />r-- Submit secondary containment test results <br />In addition, this site had an unpermitted overfill prevention drop tube installed on August 7, 2002. A permit has <br />since been issued, however, this overfill prevention device was not set at the required level as indicated in California <br />Code of Regulations (CCR) 2635(b)(2)(C), which is 95% of the tanks capacity. Immediately install an overfill <br />prevention system that satisfies CCR 2635(b)(2).
