Due to access limitations, these proposed penetrations will be installed on the outside of the <br />UDC 3/ sump. The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br />applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. <br />3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection appointments (a 48- <br />hour notice is required for inspection appointments). <br />a. 24-hour hydrostatic test (or equivalent) on the UDC % sump upon installation <br />4. Since concrete will be broken near or above fuel piping, a secondary containment test must be <br />performed on impacted piping (once concrete is poured) in accordance with the manufacturer's <br />guidelines. <br />5. Secondary containment testing for the impacted UDC 3/ sump is required six months after <br />installation. Provide 48 -hours notification to the EHD when this test is scheduled. <br />6. Qualified service technicians must possess or work under the direct and personal supervision of <br />an individual physically present at the work site who possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued by the Contractors <br />State License Board (may be issued to the installation company), or a Tank Tester's license <br />issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the International Code <br />Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br />tested, repaired, or serviced. <br />7. Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site who possesses all of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, and <br />
