The permit has been approved with the <br /> following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Vicki McCartney at (209)468-9852 to arrange the following inspection <br /> appointments (a 48-hour notice is required for inspection appointments). <br /> a. Corrosion protection: Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling. <br /> b. Spill bucket test: Verify minimum 5-gallon capacity and pass a one hour hydrostatic <br /> test.(or equivalent). Proper drainage must also be verified. <br /> Please be aware that the facility cannot receive fuel in the 87 tank prior to the EHD <br /> inspection and approval. <br /> 4. Since concrete will be broken over the 87 tank, a secondary containment test must be <br /> performed on the UST once concrete is poured in accordance with the manufacturer's <br /> guidelines. <br /> 5. . Any individual performing the work of a service technician must meet the following <br /> qualifications: <br /> a. Possess or be employed by a person who possesses one of several permissible <br /> valid, current contractor's licenses issued by the Contractors State License Board <br /> (may be issued to the installation company), or a Tank Tester's license issued to the <br /> individual by the State Water Board, and <br />
