March 20, 2015 <br /> MAR 2 5 <br /> r <br /> To Whom It May Concern: °^-71 <br /> I, Nayer Hatami,am writing this statement in regards to the inspection performed by Elena Manzo of <br /> the EHD on February 24, 2015. The following corrections have been made and submitted to CERS for <br /> approval: <br /> #106: Monitoring and response plan has been uploaded and submitted to CERS. <br /> #109: Secondary containment test was completed on July 30,2014 instead of an earlier date due to <br /> the fact that our service technician, Nick Zagarov from Eco CHEK,was very ill and was not able to do the <br /> test within the allotted time; however, Nick did inform Tui from EHD of the situation and was later <br /> resolved. Going forward,all necessary precautions to ensure testing will be taken to be performed in a <br /> timely manner. <br /> #110: A copy of the hazardous waste generators manifest has been submitted to DTSC Report <br /> Repository. Also, a copy of this manifest will be sent to EHD. <br /> #126: A complete list of all new UST information, including UST Monitoring Site Plan, UST Certification <br /> of Financial Responsibility, UST Response Plan,etc. have all been uploaded and submitted to CERS. <br /> Proof of correction will also be sent to EHD. <br /> #306: Monitoring equipment to detect leak(sensor)was corrected and is maintained to detect a leak. <br /> This was corrected on site on February 24,2015. <br /> #307: The 87-octane annular sensor monitoring system was corrected on site on February 24,2015. <br /> The sensor was replaced and retested during the inspection. A"UST Retrofit Verification with Inspector <br /> Already Onsite"form has been completed for the replaced annular sensor and provided to the operator <br /> and contractor. <br /> #309: All monitoring UDC's that utilize the Bravo box float and chain were adjusted by the technician <br /> and verified functionality to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak.This was corrected on site on February 24, 2015 <br /> #605: Anew label was prepared and affixed to the 55-gallon drum at the time of inspection on site <br /> February 24, 2015 to ensure that all hazardous waste containers are marked with all the required <br /> information. <br /> If you have any further questions, please feel free to contact me at 209-406-6003. <br /> Sincerely, Nayer Hatami <br />
