Beginning January 1, 2014, all businesses are required to submit all new(or <br /> any changes to existing) UST information, including: UST Monitoring Site Plan, UST Certification of Financial <br /> Responsibility, UST Response Plan, UST Owner/Operator Written Agreement(if applicable), UST Letter from Chief <br /> Financial Officer, and Owner Statement of Designated UST Operator Compliance online to the CERS at <br /> http://cers.calepa.ca.gov. While onsite, discussed electronic reporting requirements, and went over the forms that <br /> need to be uploaded to correct this violation. Upload all missing forms and submit proof of correction to the EHD. <br /> This is a Class II violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 91-octane STP sump sensor was located inside a sensor holder, which was located approximately 4- inches up <br /> the sump wall, and therefore the sensor was off the lowest point of the sump and not located to detect a leak at the <br /> earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible <br /> opportunity. The sensor was removed from its holder and relocated to the lowest point of the sump by the service <br /> technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This was <br /> corrected on site. <br /> 307 CCR 2632(c) Monitoring system of the interstitial space of a double walled system not maintained. <br /> The 87-octane annular sensor failed to go out of alarm after testing. The technician onsite evaluated the sensor, <br /> and recommended a sensor replacement due to inconsistent functionality. The interstitial space shall be monitored <br /> with a monitoring system connected to an audible and visual alarm system. The sensor was replaced and retested <br /> during the inspection. A"UST Retrofit Verification with Inspector Already Onsite"form has been completed for the <br /> replaced annular sensor and provided to the operator and contractor. This was corrected on site. <br /> 309 CCR 2636(f)(1) Monitoring in the UDCs failed to activate an audible and visual alarm or stop the flow at the UDC. <br /> The Bravo box float and chain in the UDC 1/2, UDC 5/6 and UDC 7/8 sumps failed to stop the flow of product at the <br /> dispenser when tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop <br /> the flow of product at the dispenser when it detects a leak. The service technician adjusted the chain(s) and verified <br /> functionality. This was corrected on site. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by March 26, 2015. <br /> Consent to perform the inspection, take photos and collect samples was given by Azam Arbabian, property <br /> owner. This facility's designated operator is Anthony Abano (ICC expiration date: May 20, 2015) . The service <br /> technician was Nick Zagorov (ICC expiration date: May 30, 2015), who had the following manufacturer's <br /> certifications: VR: March 20, 2015 and VMI: March 18, 2016 <br /> The 91-octane annular sensor failed to clear out of alarm for more than 30 minutes post testing.
