Financial responsibility documents have not been submitted to EHD. Last financial responsibility <br />document received on 02/07/2011. Financial responsibility is valid only for one year. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility (Certification of Financial Responsibility and Letter from Chief Financial Officer or <br />equivalent) by 03/19/12. Repeat violation. <br />113. New designated operators Nik Zagorov and A. Steven Abano had performed all of facility's monthly <br />visual inspection reports since May 2011 and notification was not provided to EHD within 30 days of the <br />change. Any changes shall be reported to EHD within 30 days. Submit notification to EHD identifying all <br />the designated operators for this facility by 03/19/12. Repeat violation. <br />- Received on site copy of unsigned Tank Owner Statement of DO <br />204. The designated operator failed to check and note the correct test dates for all the required testing <br />on all the monthly visual inspection reports from February 2011 to April 2011. Reports For February & <br />March 2011 by Franzen -Hill Steve Zwahlen stated no date "no records" for secondary containment <br />testing. Report for April 2011 by Franzen -Hill James Flowers stated the annual monitoring system <br />certification & secondary containment/ bucket testing as 07/31/10. , the <br />designated operator shall check that all required testing and maintenance for the UST system have been <br />completed and document the dates they were done.
