3 <br />March 15, 2011 <br />Thuy Tran <br />Re: Inspection Report 1/26/11 <br />Dear Ms. Tran: This letter is in regards to inspection on the date mentioned above. Violation #3 and #4, <br />the information is on file with EHD. Violation #5 and #6 regarding Monitoring and Response Plan, EHD <br />has a copy of the Response Plan; however, the Monitoring Plan is either with EPIC Compliance or <br />Franzen -Hill . I am not able to retrieve records from EPIC since I have an outstanding balance with them. <br />I am sure that if your office requested it from them, they should be able to provide you with one. If <br />either one of them do not have it, then I need to do one. <br />The UST facility and tank forms have been put on file at our site and a copy is provided. The UST <br />operating permit application was provided to you from myself on 2/7/11. Violation #7 has also been <br />corrected and is on site as well as a copy provided for you. <br />Violation 23a has been corrected with EHD; Violation 23c has been performed by Franzen -Hill and an <br />inspection reports are on site and have been logged properly as well as provided a copy to EHD. <br />Violation 23d has not been performed in the last 12 month. Our employees have been trained and have <br />signed the proper form and is on file at Lathrop Shell from 1/21/09.
